Event Requests

Do you have an event coming up?

There are four different options of request forms to fill out.  Please read below carefully and select the correct form for your event.

Event Request and Details Form:

This form is intended for:
  • All outside groups requesting to host an event at Faith Church
  • Faith Church Ministry Staff and Leaders planning an event that requires one or more of the following:
    • Event promotion (website, weekly email, announcements)
    • Graphic design
    • Registration setup
    • Tech support
    • Hospitality support

If you need any of these services, you must complete the Event Request & Details Form.

FAITH STAFF AND MINISTRY LEADERS ONLY - If your event is more than two months away and you do not yet have all the necessary details, please first complete the Calendar Request Form to reserve your date. Then return to submit the Event Request & Details Form once your details are finalized—no later than 45 days before your event.

Calendar Request Form:

The Calendar Request form is intended for Faith Staff and Faith Ministry Leaders only (If you are an OUTSIDE GROUP wanting to request event space at Faith Church, you need to fill out the Event Request and Details Form) -  Please complete this form only if your event falls into one of the following categories:

  • You need to reserve space on the calendar but DO NOT require any event promotion (website, weekly email, announcements), graphic creation, registration setup, tech support, or hospitality support. If you need any of these services, please submit an Event Request and Details Form instead.

  • Your event is more than two months away, and you do not yet have all the details. In this case, fill out the Calendar Request form first, and then you must return at least 45 days before your event to complete an Event Request and Details Form.
